Description
A Brief Overview Facilitates care of patients and families to appropriate and alternate levels of care as needed through collaboration with the nurse managers and physicians. Works with leadership to manage patient and family needs working to ease throughput and/or decrease/eliminate any issues. Work with all divisions and staff, including senior leadership to ensure exceptional patient/family experience. Partners with managers and leadership to assure patient/family experience exceeds expectations. Promotes Relationship Based Care and models the UH essential behaviors and core competencies during all interactions with patients, families, physicians and employees.
What You Will Do - Collaborates with Director to provide competent, efficient and caring services to members and their families
- Develop and maintain close relationships with members through initial orientation, assisting with health care needs, proactive communication, participation in call schedule once a month to provide 24/7 coverage
- Communicates with Director and Senior Leadership team necessary information to provide exceptional care.
- Collaborates to help improve systems that support patient experience including quality improvement projects
- Responsible for maintaining the environment of care and attending to the needs of the clinical staff through visibility/presence in the clinical areas, coaching and6 assisting with patient/family needs.
- Serves as the clinical information resource for care coordinators and liaisons.
- Delivers service recovery to patients and family members to ensure a positive experience.
- Develop and maintain positive relationships with physicians, hospital staff, and senior leaders.
- Schedule healthcare appointments and coordinate administrative functions
- Assist member w/ air medical travel arrangements and cultural needs
- Accompany patients/family to appointments, attend appointment, taking notes as needed and participate in clinical discussion with patient, family and doctor.
- Advocate for patient, family and staff needs.
Additional Responsibilities - Performs other duties as assigned.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications
Education
- (BSN) Bachelor's Degree in Nursing (Required)
Work Experience
- 5+ years bedside/ direct patient care RN experience (Required)
Knowledge, Skills, & Abilities
- Demonstrated leadership, management and human relations skills. (Required proficiency)
- Proven knowledge of insurance, hospital and professional regulations. (Preferred proficiency)
- Excellent interpersonal skills, physician relations experience. (Preferred proficiency)
- Extensive troubleshooting, resolution & follow up on high profile issues. (Required proficiency)
- Detail oriented with excellent organizational and analytical skills. (Required proficiency)
- Demonstrated skills in Microsoft Office & Excel Information technology skills, preferably with knowledge of Kronos Scheduling and Optilink software (Required proficiency)
- Outstanding written and verbal communication skills (Required proficiency)
- Excellent time-management skills and proactive approach to medical needs of members (Required proficiency)
- Comfortable working with many prominent and affluent individuals including physicians, executives, business entrepreneurs (Required proficiency)
- Comprehensive understanding of local medical systems and resources (Required proficiency)
- Compassionate and caring personality motivated by an opportunity to provide meaningful assistance to members (Required proficiency)
- This is a part time position - 16 hours per week. Weekend coverage is essential.
